Some users not recognized at login on virtualized foswiki

2.1.8 install on FreeBSD 11.2 running nginx 1.24.0. It serves 4 wikis using VirtualHostingContrib. Some users (not all) consistently get an "Oops: we could not recognize you. Try again or reset your password. Login UserName invalid" error message whenever they try to login. They do exist and the reset their password link works. No errors regarding the user in the foswiki logs or the nginx error logs. The users experiencing this tend to login once a week or less.

We've tried the following with mixed results:
  • Sometimes resetting their browser cache fixes it but this is suboptimal as this messes up all of their sessions.
  • Sometimes trying another browser lets them login, sometimes it doesn't.
  • Killing all of the perl processes then restarting foswiki let's them login but seems to "reset" the other problematic users so they can't login.
We suspect an issue with fcgi, caching, or some browser plugin we haven't pinned down yet. Suggestions?
